Most people with Internet and at least Adobe Reader 5 (which is free and installed on most computers) will be able to type into these online forms and save them to your hard drive.

You are not required to use these online forms.  They have been provided only as a convenience to our families.  The school office provides all these forms for those wishing to fill them out by hand - just contact the office and they will send all the necessary forms home.

You can fill in the boxes by clicking inside the gray area for each blank and typing on your keyboard.  Remember that signatures and signature dates are left blank for a hand signature.

Filling and saving the form DOES NOT send it to the school office. You may fill it out directly online, but you must print it, and hand carry it to the offices indicated.

Once you have given us your intent to register, please fill out your parish verification form and turn it into your parish office so that the Pastor, Pastoral Coordinator or their designee can verify the information, and sign and date it. Pick up the form, and return it to the school office ASAP.

Once parish verification is complete, you can turn in your contract. Please fill it out, print it and sign it. Page 1 does not change much from year to year, so you should keep a copy of it on your computer so you can glean any unchanged information for upcoming years.
