You can open these forms and fill them out with the computer. Because most forms require signatures, they cannot be automatically sent to the office. After you have filled out the forms and saved them, please print, sign, and turn into the office.

You may print these forms and neatly fill them out by hand if you prefer.

CONTRACT - use a single contract form for all students, preschool, kindergarten, and 1-8.

*** In order to open, fill out and print these forms, you must have Adobe Reader, which is a free add-in for your computer. PC's and Linux work with Adobe Reader, and MAC's that run PC programs usually do, too. However, if you have a MAC that won't open these PDF files, then come by the office and pick up a set of forms.

I've tried to store the forms so even computers with the older Adobe Readers will work.

However, if you don't have Adobe Reader (now at version 9.3) at all, you may click this link to download and install it. (You might want to uncheck the McAfee Security Scan box.) Usually, there is a yellow bar at the top of the page saying the download was blocked and do you want to allow it? Say yes, or allow. Once you have downloaded it, a popup will have a button to INSTALL NOW. After installing, you should be able to open these forms.
